Intructions

1. Print out the mail order form.

2. Fill out your delivery address(including name) and email address.

3. Find the items you would like to purchase and add them to the shopping cart on Dog-Toy.co.uk.

4. Copy the item quantity's, names and prices from the shopping cart on to the order form.

5. Fill out the sub total and total on the order form, these values are automatically calculated by the Dog-Toy.co.uk shopping cart.

6. Send your cheque and order form to Dog-Toy.co.uk, 41 Westview Avenue, Glen Parva, Leicester, LE2 9JU.

7. Thats it! expect an email telling you your items have been dispatched once we have received and processed your order.
